SUMMER CAMP NEWS - MEDICAL FORMS DUE JUNE 21ST 2007 PLEASE

Hi all,

Summer camp medical forms, Signed by a Physician, (no, this one cannot be signed
by a parent), and need to be delivered to Dianne Delaney no later than June
21st. Hopefully this is not a surprise and you are on your way to getting the
forms completed.

If, after reading the small print on the form you have questions about it, see
me. When they are completed, please see Mrs. Delaney.

The Class 3 Health and Medical Record form and the Certificate of Immunization
form can be found on line in several places, including our weblog
www.troop61.info , click under “forms”.

They must be completed and signed for Scouts and Adults age 40 and older within
the previous 12 months - from the date of the start of camp - July15th 2007. If
you’re younger than 40 and can prove it (Kidding) then see me. If you were born
before 1957, (and are willing to say so!) then you get a pass on some of the
immunizations. Otherwise, I need the forms to take with me to a meeting Before
we go to camp, so please try to meet the deadline. Thanks.

If you want your child to receive Tylenol or Ibuprophen, or other
over-the-counter medicines if he has a headache, stomach ache, etc while at
camp, etc, then please fill out and sign the Authorization to Administer
Medication to a camper form.

More on medications taken to camp and proper labeling later.

Please take care when filling out the “Parental statement” and “Medical History”
sections to be accurate and complete. If you or your son gets sick while at
camp, it’s very hard to figure out what’s wrong and how to help you if we don’t
know what medicines you’re taking, etc. Be very sure that no one other than Mrs.
Delaney, the Camp RN and me will have access to the forms, this is on a
need-to-know basis, and two of us are bound by license to respect your privacy
anyway. But each year there seems to be at least one adult or child who needs
medical attention and it is so, so much easier if we know how to help you.
Enough said.

As mentioned before, there will be a riffle range permission form coming out to
be signed for permission for your son to use the firing range, under direct
supervision as always.

And as before, if you or another adult in your family plans to stay overnight,
visit on a regular basis, or be involved in any activity of the Troop’s while at
camp, please include your information for a CORI check - which must be done
ahead of time in order to be completed in time for you to participate at camp.
The info needed is: your name, any alias/maiden name, date of birth and social
security number. If you stayed at camp last year and haven’t changed your date
of birth then you don’t need to give it again as I still have it. Otherwise,
please give this info to Mr. Helgeson when signing up for parental help at camp.
And please sign up - we can’t run camp without you!! (Besides, it’s fun).

That’s all for now. After we get the business stuff out of the way, we can
concentrate on the fun part of camp! Like merit badge classes. And the bugs. And
swimming. And the mosquitoes. And fishing. And the sunscreen.

If any of this causes you problems don’t sweat it, call me and we’ll figure it
out. And remember - it’s Camp! It’s supposed to be Fun!

Start practicing those knots - for tying up the mosquito netting over your bed
of course !!! See you soon!
